Background. Despite the significant number of existing cephalometric techniques, no single indicator is characterized by absolute accuracy and reproducibility.
Aim: to optimize the diagnosis of patients with skeletal forms of dentoalveolar anomalies in permanent occlusion through standard and additional cephalometric analysis.
Materials and Methods. Data from 24 patients regarding possible orthognathic correction were included. Cephalometric examination was performed in a standard lateral projection in the natural position of the head. Standard angles were studied: SNA, SNB, ANB. Additionally, the Palaczek angle was determined to assess the sagittal position of the lower jaw relative to the facial skeleton. To determine the diagnostic value of the Palaczek angle indicator, ROC analysis was performed. The statistical program MedCalc for Windows 14.8 (MedCalc Software Ltd, Belgium) was used. The work was carried out within the framework of the research project "Optimization of modern methods of diagnosis and treatment of diseases, traumatic injuries, deformations and anomalies of the development of the maxillofacial region in peacetime and wartime conditions" (2025-2027), state registration number 0125U000945.
The study was conducted in accordance with the ethical standards of the World Medical Association Declaration of Helsinki (1964–2024), the European Society Directive 86/609 on the participation of humans in biomedical research, as well as the Order of the Ministry of Health of Ukraine No.690 of 23.09.2009.
Results. It was determined that among 24 patients, the results of the Palachek angle improved the diagnostic process in only 3 (12.5%). Its diagnostic value has not been determined using ROC (Receiver Operating Characteristic) analysis (p=0.617).
Conclusions. Timely and accurate diagnosis of skeletal dentofacial anomalies is of fundamental importance for choosing the optimal treatment tactics, determining indications for orthodontic or orthognathic correction, and predicting functional and aesthetic treatment outcomes. Although the diagnostic value of the Palaczek angle has not been determined using ROC analysis, it can be used in the diagnosis of adult patients who are scheduled for orthognathic surgery (occlusal change surgery). Measuring the Palaczek angle can be useful for assessing the facial profile; determining the degree of retrognathia or prognathia of the mandible, but it is not one of the basic or mandatory parameters of the current planning protocol.
