Background. Despite the accumulation of experience in the treatment of combat trauma, the number of detailed clinical cases with long-term observation and analysis of all stages of reconstruction remains limited.
Aim. To describe the features of severe mine-blast trauma of the maxillofacial area and give an example of phased reconstruction of the injury.
Materials and Methods. A clinical case of multi-stage surgical treatment of a patient with severe mine-blast trauma of the maxillofacial region is presented, which included a series of reconstructive surgical interventions aimed at restoring the anatomical integrity of the facial skeleton, functional capabilities, and facial aesthetics. The study was carried out within the framework of research with the state registration number 0125U000945.
Research Ethics. The study was conducted in accordance with the ethical standards of the World Medical Association Declaration of Helsinki (1964–2024), and the Order of the Ministry of Health of Ukraine No.690 of September 23, 2009. The patient's informed consent was obtained to participate in the scientific study and publish facial photographs.
Results. The experience of surgical treatment of a patient with a mine-blast injury is presented: multiple lacerations of the face, neck, chest, left upper limb. Laceration of the cheek, parotid-temporal, zygomatic areas and left auricle. Traumatic open fracture of the left branch of the mandible. Hematosinus. The course was complicated by a salivary gland fistula. Staged reconstructive surgical correction was performed for 64 days (primary surgical treatment, necrectomy and secondary sutures, reconstructive surgery, surgical correction of the salivary gland fistula). Along with surgical treatment, bimanual splinting was used to treat fractures of the upper and lower jaw. Long-term rehabilitation measures were carried out. Maximum restoration of facial tissues and socialization of the patient were achieved, chewing and speech functions were restored.
Conclusions. The choice of surgical tactics and the results of treatment of a patient with severe mine-blast trauma of the face depended on the severity of the injury, the degree of damage to the soft and bone tissues of the face, and the functioning of the salivary fistula. Rehabilitation measures were important. The presented clinical case is an example of staged reconstruction after a mine-blast trauma of the face.
